We are excited for you to join our care family, but there are a few more steps in your journey to make sure you get the customized health plan you deserve.
Our Process
STEP ONE
Meet & Greet
The meet & greet is a 15 minute call, free of charge, to make sure that what you are looking for is a match for what we offer. In this call you can also ask questions about our unique approach. Now that you completed the online signup, our team will text your within 1 business day to setup a time for the call. If you have already had your meet & greet, Woohoo! you can skip this step.
STEP TWO
Log Into Your Patient Portal
The Direct Osteopathic Primary Care Patient Portal allows us to send you lab results, securely send and receive messages with you, review your medication history, and more.
Go to: https://mydenverdo.md-hq.com/log_in
Log in using the credentials you created from the welcome email you received. Check your email for a message with the subject “Welcome to Direct Osteopathic Primary Care's Patient Portal” (check your spam folders and text or call us with any questions 303-422-2236).
STEP THREE
Fill Out Your Questionnaires / Forms
Go to My Account Details and verify/complete information
Go to Questionnaires to complete New Patient Pre-Visit Forms
Go to My Pharmacies and add at least one pharmacy
*Pro Tip: Walgreens is usually the most expensive pharmacy, self pay options are best online at Cost Plus Drugs.
Go to My Allergies to add your medication allergies.
If you are on medications, add them to My Medications. If you are on supplements, add them to My Supplements. You may also bring medications and/or supplements to your first visit and we can add them in for you.
STEP FOUR
Schedule Your First Appointment
The last step on your journey to great care is to book your first appointment with DOPC. We have 3 easy ways to schedule an appointment
1. Send us a Secure Message the Patient Portal: This will allow you to give us details about your goals and concerns for the first appointment, along with time and availability.
2. For the fastest option: Text our office number at 303-422-2236 but please do not include any sensitive information.
3. Call Us at 303-422-2236.
*Don't forget, all of the new patient forms and questionnaires in the patient portal must be filled out 24hrs prior to your first appointment, or your appointment will be rescheduled. We want to make sure we use our time available to get to know you, not on the paperwork.
Don’t worry, you won’t pay anything until your first visit.
Once all of these steps are complete, your membership is activated!Now sit back, relax and prepare for a new paradigm of health+care
